Heartbeat Connections Program

The HeartBeat Connections program is a telephonic coaching program designed to provide one-on-one support for individuals trying to improve their cardiovascular risk factor profile. Participants are identified and invited to this program based on information in their Allina electronic health record. Specifically, eligible participants must be 56073 zip code resident, age 40-79 years, have an active Allina health record, no active vascular disease or diabetes, and at high risk of experiencing a heart disease event over the next 10 years. Approximately 1,500 individuals will be invited to participate in this program over the next year. 

Telephonic coaching sessions are led by a trained nurse practitioner, pharmacist or registered dietitian and are intended to provide added support and education between regular clinic visits to improve overall heart health. The phone appointments will focus on lifestyle changes such as tobacco cessation, weight management, increased physical activity, and healthy eating, as well as preventive medical therapies when appropriate.
