All patients eligible for the HeartBeat Connections program receive an information packet that discusses risk factors for heart disease and a resource list. The packet includes an educational handbook.

HONU Clinical Resources

Some HONU materials may benefit your practice. This program also offers a resource guide full of local resources to help your patients quit smoking, improve their nutrition, become more physically active and manage their weight. For a hard copy, please contact Betsy Pieser at 507-233-2048.
